Patients are seen by appointment only. The satellite office hours vary and are scheduled by the local clinics. Patients should call the local clinics to make appointments.

New Patient Forms

When you come in for your first visit, you will need to complete forms regarding your medical history and insurance information. You can save time by downloading these forms now and completing them at home. When you come in for your visit, don't forget to bring your completed forms.

Telephone Calls

All patients are encouraged to call with any questions they have about their medical problems. The office staff is trained to answer questions regarding ear, nose and throat. They will also relay any unanswered questions to your doctor and your call will be returned as soon as possible.

Requests for prescription renewals should be called in during regular office hours.

If you find it necessary to contact the doctor after regular office hours, the answering service will forward all messages to your doctor or the physician on call. Your call will be returned at the earliest opportunity.

Fees, Payments and Insurance

We make every effort to keep down the cost of your medical care. Our doctors are currently participating providers with many insurance plans including: Accountable Health Plans, Blue Cross/Blue Shield, First Care, Healthsmart, One Health Plan, Texas Municipal League, Texas True Choice, Tricare, TRS Care, United Healthcare, and Unicare.

For your convenience, we also accept payment by Discover, Mastercard, or Visa.

Our business office staff will be glad to discuss any financial concerns with you during regular office hours.
